Official Transcripts of all college/university course work. These are mailed directly to the University Office of Admissions. Second copies of transcripts must be uploaded in the "supporting document" section of the online application or mailed directly to the department at Public Health, PO Box 3062161, Tallahassee, FL 32306-2161.
Personal Statement:
A Short Statement of Purpose. Write a short one-page statement about your career and educational goals. Follow instructions for uploading this in the "supporting documents" section of the online application.
Recommendation Letter:
Two Letters of Recommendation. For applicants currently or recently enrolled in school, these should be from faculty. Follow instructions in the recommendation section of the online application. List the name and email address of your recommenders. These people will receive an electronic prompt about uploading their letter for your application.
